Steps on how to order:
1. Download Order Form here.
2. E-mail the accomplished form to upmcmerchcom@yahoo.com with "Orders: your name and school" as the subject. (If you are having trouble downloading the Order Form, e-mail us and we will send you a copy.)
3. Deposit the FULL payment to the UPMC bank account which will be given to you in reply to your order. Keep the deposit slip.
4. Send a message to +639178297374 or e-mail us at upmcmerchcom@yahoo.com on the day of payment to confirm receipt of payment.
How to claim:
Orders can be claimed anytime at our booth during the event and up to one week after.
1. Bring the bank validated deposit slip.
2. Bring at least one valid ID.
Deadline for orders is on February 1, 2008 (Friday).
1. Submitted orders are considered final and no alteration, cancellation or reimbursement shall be made.
2. Should we fail to produce your ordered good, a refund shall be granted upon the presentation of a validated deposit slip.
3. We would like to remind you that these are limited items so we encourage you to place your orders as early as possible. We may only process prepaid orders and these items may not be available on the day of the event itself.
